How do I make a referral?
If you would like to self-refer to our Therapeutic Services, please fill in the form below, ensuring that it is dated and email it to firstname.lastname@example.org.
Alternatively, if you'd prefer to do this over the phone we can arrange for someone to give you a call and go through the form with you. Please email your request along with your contact details & preferred availability to . A member of the team will then be in touch in due course. Please be aware that this call will take roughly 20 minutes. Before referring, please make sure that you live in an area that we cover. Click here to find out more.

What happens next?
When we receive your referral it will be reviewed by a practitioner. When we review referrals we think as a team about what may be the most useful support for you and to consider whether we are the appropriate service to meet your needs at this time. During the screening process we will consider whether your current circumstances are likely to be able to engage and benefit from our therapeutic services.
We may need to contact you or other services/professionals before considering we are an appropriate service, if so we will put you on a waiting list. If you have been advised to come her by another service or professional we will need to contact them and ask them to send a referral.
If we feel that we are the right service to support your needs you will receive a letter to let you know you have been added to a waiting list for a face to face assessment to further establish your needs. Due to the demands placed upon our services there are long waiting lists for assessments for all of our therapeutic services. If we don’t feel we are the right service at this time we will write to you to let you know.
Please be mindful that 1:1 counselling is not the only intervention we offer and based on the information you provide us with we may feel that a group is the more appropriate intervention for you.

How do I make a referral?

Prior to making a referral to our Therapeutic Service please consider:
Whether the client has the emotional resilience to engage in a structured therapeutic intervention.
Whether the client’s circumstances are sufficiently stable to support regular and meaningful attendance as well as positive therapeutic outcomes.
Whether the client has Mental Health needs beyond the mild to moderate range that we are able to work with. Please bear in mind that Yellow Door is not resourced to manage significant or chronic risks such as those relating to ongoing suicidal ideation or to self-injurious behaviours that are severe and/or repeated.

At the point of referral, we can provide estimated waiting times but please be aware that each of our therapy services operate with a waiting list system. Where Yellow Door has started therapeutic interventions and a risk profile escalates and/or complexity becomes apparent such that we consider Yellow Door interventions to be contra-indicated, we will work with appropriate services to plan transition.
At this moment, unfortunately we are unable to offer services at to under 11's at the moment, temporarily.
